Nature of the fault and has it happened before?

General Nature of the problem (e.g. damp in wall/leak from pipe/ boiler not working etc)

details and severity of the problem (e.g. one drip per 10 seconds/damp patch on ceiling 50cm wide) please provide as much detail as possible

where the fault is related to heating/hot water/appliances please provide the make and model of the system and its location

please explain how the problem occurred (e.g. accidently broke key in the lock when opening front door)

What if anything has been done to prevent further damage? (e.g. put a bowl under leak, turned off water supply etc.)

please estimate how quickly the issue needs resolving (7 days/24-48 hours/ when a tradesmen is next available)
